Adults Occupational Therapist – Discharge to Access Team

We are currently recruiting an experienced Adults Occupational Therapist to join the Discharge to Access (D2A) team within Adult Social Care services in Slough.
This role is vital in supporting timely and safe hospital discharges by carrying out holistic assessments in the community and ensuring appropriate care and equipment are in place to maintain independence at home.

Key Responsibilities:
Complete strength-based, person-centred assessments post-hospital discharge
Prescribe suitable equipment and recommend adaptations where needed
Carry out manual handling assessments and develop care plans
Liaise with hospitals, care providers, and community services to ensure smooth transitions
Enable individuals to regain independence and reduce the need for long-term supportRequirements:
HCPC registered Occupational Therapist
Previous experience in a Discharge to Assess or similar role
Strong understanding of hospital discharge processes and community care
Ability to work flexibly and independently in a fast-paced environment
Knowledge of the Care Act 2014 and relevant legislationWhy Join Us?
