Sanctuary Personnel is seeking an experienced Band 6 Community Psychiatric Nurse (CPN) to join a Dementia Service on a locum basis.

You will provide high-quality care to adults with dementia and related cognitive conditions, supporting assessment, care planning, risk management, and multi-disciplinary collaboration. The role involves community-based work, so confidence working independently and managing a caseload is essential.

Key Responsibilities of the CPN

Conduct comprehensive assessments and review care plans

Deliver specialist nursing interventions to individuals with dementia

Manage and prioritise a community caseload

Work collaboratively with MDT members, carers, and external services

Monitor mental and physical wellbeing and respond to clinical risks

Maintain accurate records and documentation

Requirements of the CPN

NMC registration as a Mental Health Nurse

Experience working within dementia or older adult mental health services

Strong risk assessment, communication, and caseload-management skills

Ability to work autonomously in the community
