     Support Navigator (Mental Health)
We are looking for 2 Support Navigators to work in a mental health support organisation in Bristol. Customers are people with complex needs living in supported accommodation.
In terms of salary, this would be £12.75 per hour.
Hours will be 37.5 hours.
There is an option of a weekly 12 hours sleep in shift which is paid at £12.21 per hour.
With a sleep in shift the total hours would be 47.5 hours per week (35.5hrs base paid at £12.75 and 12hrs sleep in shift paid at £12.21). Total salary for that would be £31,155.54.
Role Responsibilities:
  * Oversee the case load of service users you are responsible for to ensure all aspects of support are delivered, risks assessed and reviewed and outcomes met.
  * Complete and review support plans with clear outcomes and actions that enable our service users to move towards independence and belonging.
  * Promote independence and provide general support to our service users
  * Complete and review clear task plans, ensuring they are followed, actioned and recorded by yourself and other members of the team.
  * Complete, action, and review risk management plans whilst delivering creative, flexible and, outcomes driven support.
  * Support the implementation of service users' personal goals and ensure monthly KPI targets are met and the cases you support are compliant.
  * Support with crisis mitigation and resolution.
  * Report on client non engagement and create strategies to improve engagement.
  * Actively communicate with external healthcare professionals involved in clients support and understanding their roles and keeping them informed.
  * Build strong relationships with family members and be the point of contact whilst positively representing the organisation.
  * Enable service users to access and participate in their local community, including access to education and employment opportunities.
  * Define & oversee individual and communal budgets and finances.
  * Raise safeguarding referrals when required with relevant local authorities.
  * Support new staff with induction and shadowing
  * Provide help with the upkeep of a client’s property
  * Carry out any other duties as required to support the team and our service users.
Requirements:
  * Experience in mental health support
  * Relevant qualification or training
