Job title:   EHCP & Transition Officer

Location:  Bower Lodge School, Dibden, SO45 5TD

Salary:      £12,320 - £14,560 per annum dependant on experienced and qualifications

Hours:      21 hours per week; Covering 3 days per week Monday - Friday 09:00 - 16:00

Contract:  Permanent; Term Time Only

We have an exciting opportunity to join New Options Primary School - Bower Lodge School to join us as a EHCP & Transition Officer

About the role

The role of the EHCP & Transition Officer will be to organise and manage the annual review of Education Health and Care Plans for all pupils. The EHCP & Transition Officer will work closely with the Education and Admin team to ensure that school is fulfilling it’s statutory responsibilities in line with the SEN Code of Practice 2014. 

This role will require continuous liaison with key staff within the school, learning providers and multi-agencies to ensure pupils are receiving maximum support to reach their outcomes

Act as a liaison for the families and advocate for the students by attending Annual Review Meetings , minute annual reviews scheduled, and distribute to relevant colleagues and external parties in coordination with the Headteacher

Monitor and track when changes are required on EHCP’s and follow up with SEN Teams as and when necessary and attend Parents Evening, Target Setting Evenings and School Open Days to be a point of contact regarding EHCPs, Annual Reviews and Transitions and support the Leadership Team as required. 

Create reports and statistics for internal and external use, using the schools information systems and assisting with developing or improving systems. 

Work with the clinical team to assess the educational needs of young people in their final transition year, so that appropriate levels of support can be planned and implemented within agreed time-scales. 

Essential:

GCSE, or equivalent, in English, Maths and Science

Ability to compose clear, accurate reports

Experience of working with highly confidential information

Good organisational skills

Excellent communication skills

Computer literate

Commitment to the values of the organisation

Our New Options Primary School - Bower Lodge located in Dibden, Southampton has a capacity for 40 pupils catering for autistic young people aged 4 – 11.
