? The Role As a Senior Social Worker, you will:

Complete Care Act assessments, develop person-centred support plans, and carry out regular reviews.
Promote independence, manage risk, and uphold individuals’ rights and choices.
Lead on safeguarding enquiries and undertake Mental Capacity Act (MCA) assessments.
Work collaboratively with families, health professionals, and care providers to coordinate effective support.
Respond to crisis situations, including hospital discharge planning and assessing eligibility for services such as NHS Continuing Healthcare.
Maintain accurate case records, work within key legal frameworks (including the Care Act and MCA), and actively participate in supervision, reflective practice, and ongoing CPD.? What We Offer
