Health & Social Care Officer
Salary: £15.66 per hour

About the Role: As a Health and Social Care Officer (HSCO), you will play a vital role in supporting vulnerable adults across Nottingham. You’ll act as the first point of contact for citizens and professionals, helping to assess needs, manage risks, and guide individuals toward the most appropriate services. This is a dynamic, fast-paced role that requires strong communication and triage skills, often dealing with complex and unpredictable situations.
Key Responsibilities
Act as the first point of contact for Adult Social Care referrals via telephone and online.
Triage and assess incoming referrals to identify needs, manage risks, and ensure safe, appropriate decision-making.
Provide accurate information and advice to citizens and professionals.
Support individuals and organisations to access the most suitable services and support networks.
Coordinate referrals efficiently and effectively to ensure timely responses for vulnerable adults.
Maintain accurate records of calls, referrals, and assessments using internal systems.Experience & Knowledge
Sound understanding of Adult Social Care services and pathways.
Experience working with vulnerable adults or in a frontline support role (desirable).
Familiarity with telephone triage or support helplines is an advantage.
Ability to manage challenging conversations and sensitive situations.Skills & Competencies
Strong interpersonal and communication skills (both verbal and written).
Excellent listening and information-gathering abilities to support accurate triage.
Confident decision-maker, particularly under pressure or with limited information.
Ability to prioritise workload and manage multiple tasks effectively.
Strong organisational skills and attention to detail.
IT literacy, including the ability to use databases and manage online referral systems.
Empathetic, respectful, and customer-focused approach to service delivery.
