Adult Social Worker - Integrated Managed Care (IMC)

Are you an enthusiastic practitioner with a passion for adult social care? Bolton Council is looking for dedicated individuals to join our Integrated Managed Care teams. We want someone who understands that social work is about supporting aspirations and enhancing quality of life.

In this role, you will be a key player in helping adults maintain their autonomy and stay connected to their communities.

The Role: What You Will Be Doing

Working within our designated IMC services, you will provide vital support to adults with varying levels of need.

* Care Act Assessments: Undertake assessments and reviews of need under the Care Act 2014.

* Support Planning: Create tailored plans that link individuals into universal and voluntary services.

* Complex Casework: Support colleagues with complex assessments, including mental capacity and best interest decisions.

* Collaborative Practice: Participate in inter-disciplinary team working for a holistic approach.

* Relationship Building: Develop and improve relationships with service users and carers.

Requirments

We are looking for a Social Worker who is as comfortable working independently as they are within a multi-agency framework.

* Expertise: Strong knowledge of the Care Act 2014 and the Mental Capacity Act.

* Qualifications: You must be a qualified Social Worker registered with Social Work England.

* Mindset: A proactive approach to problem-solving and a commitment to promoting independence
