Overview
We are currently seeking a compassionate and skilled Rehabilitation Support Worker to provide structured evening support for a young gentleman living with an acquired brain injury and polytrauma. Your role will focus on supporting his daily routines, from meal preparation to fostering independence with everyday tasks, and offering social and emotional support to help him unwind safely and positively each evening.

Responsibilities

* Support the client with meal planning and preparation, encouraging independence and healthy choices.

* Facilitate engaging activities such as watching films or playing darts during the evenings.

* Encourage and maintain daily living routines, helping with household tasks where appropriate.

* Provide emotional and social support to help the client relax after the day.

* Monitor and report any changes in mood, motivation, or physical wellbeing to the case management team.

* Keep accurate, detailed notes at the end of every shift.

* Collaborate with the case manager and therapy team, following rehabilitation plans and guidance.

* Advocate for the client’s needs and preferences as required.

* Maintain safety, dignity, and privacy at all times.

* Attend supervision and training sessions as needed.

* Assist with transport to and from appointments and commitments.

* Perform light cleaning duties around the home.

Experience

Essential:

* Full, clean driving licence with access to your own vehicle.

* Clear DBS check.

* At least 1 year of experience in a care or support role.

* Reliable, empathetic, and confident working independently.

* Excellent verbal and written communication skills.

* Ability to engage positively with a young adult, using humour and motivation.

Desirable:

* Experience or knowledge of acquired brain injury and/or polytrauma.

* Understanding of fatigue management and behaviours following brain injury.

* Flexibility to occasionally cover other shifts during staff absences
