Exciting Opportunity: Pathway Tracker ?

Are you ready to make a difference in patient care? Our client is looking for a dedicated and enthusiastic Pathway Tracker to join their team on a temporary full-time basis asap to March 31, 2026. If you have a passion for healthcare and a knack for organisation, this role is perfect for you!

Key Responsibilities:

Patient Journey Management: Proactively track patients from referral to treatment, ensuring a seamless experience through efficient management of waiting lists and progress monitoring.

Target Monitoring: Ensure adherence to national targets, particularly the 18-week Referral to Treatment (RTT) standard. Validate data and identify potential breaches to maintain high standards of care.

Data & Reporting: Maintain accurate patient records using electronic systems (such as PAS), ensuring data quality for audits and reporting. ?

Liaison & Escalation: Collaborate closely with clinicians, secretaries, and administrative staff to resolve bottlenecks. Don't hesitate to escalate issues when interventions are needed! ?

Coordination: Arrange diagnostic tests, appointments, and admissions to keep patient pathways moving smoothly.

Meeting Support:

Facilitate Multi-Disciplinary Team (MDT) meetings, ensuring smooth operations and accurate recording of decisions.

Essential Skills & Experience:

Excellent organisational skills and meticulous attention to detail. ?️
Strong communication and interpersonal skills to engage effectively with diverse teams.
Ability to work independently, prioritise tasks, and tackle problems with confidence. ?
Familiarity with electronic patient administration systems (PAS) and proficiency in MS Office (especially Excel).
Understanding of medical terminology and targets/guidance.
