We are looking for a highly motivated and experienced Senior Clinical Pharmacist to join our progressive healthcare team in Reading. This is a pivotal role for an individual who is passionate about improving patient outcomes, advancing clinical practice, and leading innovation in medicines management within primary care or integrated care systems.

 As a Senior Clinical Pharmacist, you will provide strategic clinical leadership across prescribing, medication safety, and clinical governance. 

Key Responsibilities:

Lead on complex medication reviews and medicines optimisation for patients with polypharmacy, frailty, or chronic conditions
Provide clinical leadership on prescribing initiatives and quality improvement projects
Support and develop junior pharmacists and pharmacy technicians
Lead the implementation of local and national prescribing guidelines and formularies
Act as a key advisor on medicines-related queries from clinicians and patients
Collaborate across multidisciplinary teams, including GPs, nurses, and allied health professionalsRequirements:

GPhC-registered pharmacist with substantial post-registration experience
Independent Prescriber qualification is essential
